Medical Release Forms & Waivers
2010 Medical Release
Students must have a release form to attend any of the off-campus events offered through Shiloh Community Church. Please click here to download and print the 2010 release form. If your student is planning to attend more than one event, please check the box on the form to authorize the release for the entire 2010 calendar year; otherwise a new form will need to be completed for each event.
2010 Student Driver Release
If a student is planning to drive other students to an event, the student driver and all of the student passengers must have this form signed by their parent or guardian and turned in before they leave for the event. Click here to download and print this release form.
